Most adults take 15-20 minutes. The paediatric form takes around 20-25 minutes as it covers your child's full history including pregnancy and early life. Please do not rush it. The detail is what makes the plan useful.
It depends on complexity. If your child is under active specialist management for a serious condition, a full consultation is more appropriate. If they see a paediatrician periodically but are otherwise GP-managed, complete the form and I will assess.
